What are the responsibilities and job description for the Social Worker-Organ Transplants (Per Diem) position at RWJBarnabas Health?

JOB SUMMARY:

Provides clinical social work services to transplant candidates, recipients, living donors and their families in a multidisciplinary team approach. Provides psychosocial assessment, counseling, crisis interventions, referral to community agencies, education, bio-ethics consultation and complex discharge planning coordination. Works with patients and families in a compassionate, culturally competent, age appropriate manner in order to facilitate access to transplantation and continuity of care throughout the transplant process. Participates in all Transplant Department interdisciplinary patient care meetings as well as outreach initiatives designed to enhance patient and community education about transplantation. Responsibilities include activities in pre and post transplantation time study completion is required.

EDUCATION:

  • Master s Degree in Social Work from an accredited school.

REQUIREMENTS:

  • Certification/licensure as required by NJ state law.
  • Two years experience in medical social work, preferably in transplant and/or dialysis setting.
