Distinguishing Features: The Case Manager reports to the Counselor and Shelter Coordinator focusing on the youth in shelter providing intensive case management services such as advocacy, outreach, planning, and linking with other appropriate services in the community. This is a very fast-paced position and the person in this position must be very flexible with schedule changes as the needs of the clients are ever changing.

Requirements for The Job:

Must have a valid NYS Driver’s license and driving record (a copy of both will be required). A copy of car insurance to include liability limits. A current physical and an update to be submitted annually. Background checks will be conducted.

Job Duties: include the following. Other duties may be assigned.

· Interviews the clients in crisis situations involving social, emotional, financial, marital, health, or other problems to develop background and details of problems. Provides Crisis Intervention as needed.

· Assesses and evaluates cases and prepares and implements case management service goal plan.

· Case manages clients, assisting them to identify and resolve problems and make effective use of resources.

· Maintains working relationships with staff of other agencies and institutions, homes, and facilities, and acts as liaison between clients and agency or institution.

· Works collaboratively with community agencies and develops and maintains contact with other agency staff and local service providers to develop specialized services for clients.

· The Case Manager will attend all appointments to advocate and teach clients how to navigate through difficult systems such as the Department of Social Services and/or Child Protective Services.

· Provides consultation to staff on specific cases involving clients.

· Provides transportation to clients. Assists with moving clients.

· Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required.

· Participates in all staff meetings and staff trainings.

· Required shelter on call on a rotating basis with other staff.

· Complies with all grant requirements.

· Prepares necessary reports for grants and the agency.

· Complies with Safe Inc. standards for performance.

Outreach Duties:

· Contact with prospective client in jail and with the Court System (as needed).

· The Case Manger will escort/advocate at the Department of Social Services as needed.

· Represent Safe Inc. at table events during the day hours.

Additional Duties:

· Responsible for log books – must ensure that recording is accurate.

· Scheduling clients for outside agency activities (i.e. medical, housing, educational, food pantry, vocational programs, etc.)

· All written correspondence regarding clients must be saved/copied to their file.

· All pertinent information on clients is updated in the client’s folder.

· All client confidential information must be maintained in designated locked file cabinets.

· Attend meetings related to Safe Inc.’s population.

· Keep abreast of new developments and on-going information in the field of youth a-risk.

· Be capable of giving oral presentations on Safe Inc.’s programs.

· Responsible for following up on clients who leave shelter.

· Weekly meetings with other staff to share information regarding clients.

· Must be available for relief shifts (as needed).

· Any other duties as specified by the Executive Director.

Supervisory Responsibilities

· This position has no supervisory responsibilities.

Qualifications: Education and/or Experience

· Bachelor's degree with experience working with at risk youth.

OR

· Associate Degree with three (3) years’ experience working in a Youth Service Program.
