The Day Habilitation and Community Habilitation Provider serves as a role model and advocate and ensures individuals safety by providing consistent supervision and observation. The Day Habilitation/Community Provider promotes a nurturing learning environment in the Day Habilitation Without Walls and in the community.
Duties & Responsibilities:
The Day Habilitation and Community Habilitation Provider ensures individuals safety by providing consistent supervision and observation. The Day Habilitation and Community Habilitation Provider assists in the development, implementation and documentation of Individual Habilitation Plans, Life Plans and Day/Community Habilitation Plans. They perform tasks on-site and in the community that involves training individuals who have a developmental disability in various life skill areas, that promote fuller personal development, enjoyment and expanded independence, community integration, productivity and individualization.
Education & Experience
- High School Diploma or GED
- Valid (non-restricted) Driver’s License
- One (1) year of developmental disabilities experience, required
Physical Requirements:
While performing the duties of this job, the employee is regularly required to communicate clearly and effectively with staff and individuals. All SCIP-R physical interventions (Perform Strategies for Crisis Intervention and Prevention as necessary for physical restraint and control of individuals exhibiting aggressive behaviors including one-person escort, standing wrap, seated wrap, and one-person take down onto side).
