What are the responsibilities and job description for the Family Nurse Practitioner position at SalusCare, Inc?
Independently obtains health histories and performs physical assessments on a selected patient population.
2. Effectively provides medical monitoring through tracking of medical problems and lab values until resolution. Practices safety, environmental and/or infection control methods.
3. Treatment recommendations, initiation of treatment and referrals for care are made in accordance with best practices. Documentation accurately reflects implementation of these standards.
4. Effectually conducts evaluation of medical conditions and health management follow-up visits; evaluates response to treatments and responds with appropriate interventions.
5. Dependably evaluates for side effects and toxic reactions, with appropriate actions.
6. Continuously maintains appropriate protocols and reliably participates in regular supervision with protocol physician(s). Notifies attending or on-call physician, as appropriate, for abnormal, unusual or unexpected events/problems requiring direct physician consultation.
7. Unfailingly prescribes medication in accordance with SalusCare formulary and effectively informs patient and/or family and support system of risks/benefits and potential side effects.
8. Consistently maintains accurate, complete and concise electronic documentation (assessments, treatment plans and reviews, progress notes, etc.) that is complete and filed within established time guidelines.
9. Constantly displays cultural competence and sensitivity in patient care delivery. Ensures services provided incorporate patient-specific cultural values, reflected in assessments, treatment planning and direct patient care.
10. Meaningfully participates in treatment planning and case staffing session as needed. As appropriate, provides medically significant treatment information to family members.
11. Routinely demonstrates a sound understanding of and commitment to complying with CARF and other regulatory standards for Detox, Adult Crisis Stabilization Unit, Children’s Crisis Stabilization Unit/Addiction Receiving Facility, Transitional Living Center, and Vince Smith Center.
12. Effectively interfaces with other internal and external treatment professionals, as appropriate, to coordinate patient care.
13. Continuously remains current on programmatic issues, medical standards and performance improvement strategies within the department.
14. Reliably partakes in weekly staff meetings and/or reviews meeting minutes and handouts to remain current with medical and pharmacy practice standards.
15. Regularly participates in committees and performance improvement activities within the programs and agency. Routinely participates in Peer Review process.
16. Thoroughly provides education to patients about medications, health issues, etc. Shares knowledge of current principles, methods, and procedures for the delivery of medical evaluation, diagnosis and treatment in the area of medical and health concerns.
17. Consistently maintains compliance with all mandatory training and minimal hours of in-service training required by SalusCare and licensing agencies. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
18. Enhances internal communication and networking throughout SalusCare. Communicates and interacts effectively with other staff within the company.
19. Steadfastly facilitates and promotes a team which reflects the vision and mission of the company. Steadily interacts with co-workers and patients in a professional and ethical manner.
20. Unfailingly demonstrates flexibility and willingness to work with a varied patient population.
21. Promptly attains/maintains competence with company electronic heath record and uses it within required timeframes.
22. Successfully completes other related duties as assigned by supervisor.
Additional Duties
1. Efficiently provides medical review for Emergency Services evaluation of admission medical criteria, as needed.
2. Routinely supports and educates the Detox and CSU nursing staff with regard to medical questions, concerns and other related issues that may come up on the Acute Care Units. Assists with Medication Error/Incident Report training, guidance and counseling to nursing staff.
3. Answers and effectively responds to calls from nursing staff for the Detox, TLC, and VSC departments, even if physically working at another Acute Care facility.
4. Routinely assists with making calls to insurance companies as a medical SalusCare representative for peer reviews of a patients stay.
Requires as MS degree in Nursing. Must possess a clear/active FL license as an Advanced Practice Registered Nurse. ANCC or AANP certificate as a Family Nurse Practitioner required. One year of APRN practice experience strongly preferred. Active DEA certificate within 3 months of start date required. Must be able to attain competency with electronic health record and use it effectively within required timeframes.
