What are the responsibilities and job description for the Patient Care Navigator position at SBH Health System?
The mission of the Nursing Department is to deliver comprehensive, compassionate, and quality patient care. The mission is consistent with the hospital's mission, which is to deliver a wide variety of community oriented health services / programs.
Some Duties include :
Performs related clerical duties to ensure continuity of patient care.
Assist patients to make consult, follow-up appointments.
Makes reminder calls to patients prior to their appointment. Follow-up with patients who missed clinic, radiology and lab appointments.
Assist with patient triage as needed.
Fill out requested forms as needed
Assist with prior Authorizations
QUALIFICATIONS :
- Education Required - HS Diploma or Equivalence
- License / Certification / Registration(s) Required - Graduate from an Approved Medical Assistant Program
- Experience / Skills Required
- Previous experience in a medical setting or 2 years relevant work experience preferred
- Excellent communication / interpersonal skills
- Bilingual preferred
- Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
