Position Summary:
The Community Outreach Liaison directs community outreach activities for Progressive PHP which result in maintaining and improving customer relationships. To provide community education. To initiate and develop alliances with community resources and referring agencies.

Required Knowledge, Skills, Licensure, Training & Travel Requirements (if applicable):
Education: Minimum of a bachelors degree in related field or equivalent years of experience in the field.
Work experience: Working in mental health and/or substance abuse field preferred.
Knowledge, Skills & Abilities: Knowledge of community resources and multiple levels of care models; high level presentation and communication skills. Organizes and prioritizes work.
Interpersonal Skills: Effective problem-solving skills, and ability to work with minimal supervision. Displays self-direction by seeking solutions to problems. Seeks out and offers new ideas for improving processes. Deals constructively with stress.
General Performance: Displays willingness to help others. Travel Requirements: Travel to referral sources and community outreach expos.
Essential Job Duties

Develop referral relationships with resources in the community, physicians, therapists, businesses and industries, managed care, etc.
- Maintain daily contact with referral sources to ensure adequate flow of communication via personal visits and telephone as set by weekly schedule.
- Organize and coordinate public presentations, workshops, and other opportunities for Progressive PHP to gain community exposure.
- Communicate and document referral development efforts and report to Program Director
- Offers ideas for new strategies and/or ways to improve our referral sources.

Coordinates, maintains, and analyzes referral database system to communicate and document the pertinent data regarding referral source/contacts.
- Prepare comprehensive quarterly community outreach plan and assist in the implementation and evaluating of community outreach strategies.
- Maintain information to the Program Director regarding referral source needs.
- Complete monthly report of referral contacts and outcomes to Program Director.
- Coordinate with Program Director as to community outreach plan for the month.

Community education: obtain inquiry information and provide other services as requested by referral sources and community resources.
- Provides education and community outreach support groups on mental health and chemical dependency to various community organizations and referral sources as requested.
- Organizes and promotes educational workshops.
- Inform Program Director of requests from all communities in need of our services.
