Securing Resources for Consumers Inc. has been serving individuals in the Durham area since 2006. Since our founding, we have consistently supported individuals with developmental disabilities and/or mental health diagnoses living in their homes with their families, or in other group homes and placements.

We are currently seeking a dedicated and compassionate individual to join our team as a Qualified Professional.
Responsibilities
- Perform comprehensive functional assessments and reassessments which integrate symptomatology and treatment recommendations into plans of care
- Develop appropriate plans of care or person centered plans to assist clients with identified outcomes and objectives. Ensure short-term and long term intensive service plan implementation, including authorization of specific in home and community services to meet the identified client needs
- Resolution of client directed concerns to ensure quality service delivery
- Collaboration and coordination made with other providers of client services
- Discharge, terminate, or transfer as needed to ensure optimum care
- Adherence to all agency policies including ethics, service regulations, and professional practices
- Collateral contact made with therapists, and other personnel for case consultation, service plan development, and provider oversight
- Maintenance of client records
- Maintenance of community or facility based Individualized Supervision Plans
- Coordination of services with other community service providers including community mental health and disability service organizations
- Attend all trainings and in-services as required for the position
- All other duties as assigned per Service Specific Definitions
- Knowledge of local in-home and community options and resources for children, adolescents, seniors or adults with mental health diagnoses and their caregivers
- Direct functional assessment, service planning and implementation experience
- Experience monitoring client service plan implementation regarding; service delivery quality, timeliness, and appropriateness
- Ability to function in a multi-lingual, multi-cultural environment, including providing service with the use of interpreters
- Ability to travel to and from client’s home that may not be ADA accessible
- Excellent oral and written communication skills
- Experience in crisis intervention
- Ability to maintain paper and electronic records and files of clients and services provided
- Experience in using computers and software including Microsoft Office products
- Ability to establish and maintain effective working relationships with clients, families, caregivers, diverse service provider networks, medical personnel, and agency staff
- Ability to work independently in the field and effectively as a part of a team and follow the agency’s values
- Ability to work under pressure, within short timelines to implement service plans
- Must be willing to see clients at home or in the community
- Must have a valid driver’s license and insurance
Qualifications
- Minimum education requirements (Bachelors in non-human services field and 4 years of MH/DD/SA experience; Bachelors in human services field and 2 years of MH/DD/SA experience; or Masters in human services field and 1 year of MH/DD/SA experience)
- Possession of a valid driver’s license
- Use of a reliable motor vehicle
- Proof of mandatory state motor vehicle insurance
- Ability to pass a criminal background check
- Degree verification
- Understanding of systemic barriers and social determinants of health
