The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.
PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose
To ensure safe, dependable, efficient transportation services to participants of Senior Care Partners P.A.C.E. This position requires constant interaction with caregivers, van services, and community agencies. Maintains a good partnership with our transportation services.
Duties and Responsibilities
- Participates in all initial and ongoing assessments as a representative of Senior Care Partners P.A.C.E. transportation discipline.
- Identifies and develops a specific plan of care for the transportation needs of the Senior Care Partners P.A.C.E. participants ensuring Senior Care Partners P.A.C.E. policies.
- Participates in the planning and collaboration regarding the use of Senior Care Partners P.A.C.E. transportation services to and from all internal (center) and outside provider services.
- Communicates and coordinates daily with the Clinic Schedulers/Referral Coordinators to ensure transportation is set up for upcoming appointments for participants.
- Calls participants to inform them of upcoming appointment pick up times.
- Communicate between vendors to ensure transportation needs of participants are met.
- Follows established schedules and adjusts schedules to meet unanticipated changes for participants.
- Participates in IDT meetings as a representative of Senior Care Partners P.A.C.E. transportation discipline.
- Documents in the participant’s medical record as required by policy.
- Must demonstrate consistent involvement, support, and collaboration with the other staff and the participant’s caregivers by using the Participant Centered Care Model.
- Responsible for completing all required education and file requirements on time.
- Keeps Leader informed of problems or issues.
- Performs other duties as assigned.
Qualifications
- Must possess, as a minimum, a high school diploma, or equivalent.
- One to two years scheduling experience preferred.
- Detail oriented with good problem-solving skills.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Excellent organizational skills.
- Strong communication skills, both written and verbal.
- Must be knowledgeable of computers, data entry, output, and system applications, etc.
- Minimum one year’s experience working with the frail elderly or long-term care population.
Work Environment
- Subject to frequent distractions and interruptions during daily tasks, requiring flexibility and the ability to quickly adapt to changing priorities.
- Communicates with staff, participants, caregivers, and other organizations, as necessary.
- Works beyond normal working hours when necessary.
- Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
