POSITION DESCRIPTION
TITLE: Caregiver Support Specialist
REPORTS TO: Case Coordination Unit Assistant Director
POSITION SUMMARY: Provides support, service coordination in assessing the needs of caregivers and receivers, and linkage to appropriate services.
QUALIFICATIONS:
1) Experience in Social Services or another related field preferred.
2) Interests in needs and welfare of older adults
3) Ability to relate to older persons and their caregivers
4) Ability to relate to staff in a professional manner
5) Positive cooperative attitude.
6) Ability to assess needs of older persons.
7) Willingness to serve as an advocate for client’s rights.
8) Willingness to travel while on the job.
9) Possess a valid driver’s license in the state of Illinois.
10) Access to a vehicle in good mechanical condition and maintain insurance.
11) Ability to follow instructions.
12) Ability to keep records and independently manage time.
RESPONSIBILITIES:
1. Participates in the Family Caregiver Programs, such as Respite, Counseling,
Gap Filling, Grandparents Raising Grandchildren and is knowledgeable of the
Community Care Program (CCP), Managed Care Organizations (MCO) and other special projects.
2. Assess client needs including physical, social, health and environmental needs and how these interact by completing in-home assessments.
3. Targets services to those aged 60 and over, who have a family caregiver in need of support services.
4. Connects individuals to available services and programs in their community which they may qualify.
5. Encourages and assists persons to gain access to needed services, including: a) Making referrals and providing on-going follow ups to ensure services are in place. b) Assist in completing all necessary forms for state and federal benefit programs c) Advocate on behalf of client to gain access to benefit programs.
6. Maintains case notes and develops a person-centered care plan for each client.
7. Advocates for clients in problem situations and makes all attempts to meet client’s needs including through alternate resources.

8. Accepts case assignments as given with due consideration to case load, mileage, and time guidelines.
9. Completes TCare Assessments on Caregivers
10. Attends necessary training as provided by the Illinois Department on Aging.
11. Participates in case conferences, case planning and implementation, coordination of resources, ensuring alternate services and monitoring client’s progress.
12. Assess clients for eligibility for access to additional programs like, home delivered meals.
13. Completes daily confidential charting for clients.
14. Participates in quality reviews.
15. Coordinates with other agencies as needed.
16. Cultivates community liaison relationships.
17. Expands the service resource network.
18. Serves as resource person for persons aged 60 and over and their families in the community.
19. Provide education to caregivers and care receivers.
20. Facilitate classes, workshops and/or support groups with focus on the dementia friendly community initiative.
21. Attend Caregiver Support Program meeting with AgeGuide.
22. Keeps statistical client data necessary for appropriate records and reports to manager.
23. Projects good image for agency.
24. Maintains credibility and integrity of organization.
25. Performs all other duties as assigned.
Job Type: Full-time
Pay: $40,000.00 per year
