Job Responsibilities
- Provide Comprehensive Support: Offer case management services, including intake, assessment, crisis intervention, advocacy, referral, and monitoring of families to ensure they receive the support they need.
- Assess Family Needs: Gather social history to assess family strengths and weaknesses, as well as existing problems, to develop effective support plans.
- Conduct Home Visits and Office-Based Services: Visit clients in their homes and provide office-based services to accommodate their needs and provide necessary support.
- Care Coordination: Collaborate with outreach workers, health educators, nutritionists, and other service providers to prevent service duplication and ensure high-quality care for individuals served.
- Service Planning: Identify and procure group services that cater to the individual's needs, maximizing their ability to live outside an institution.
- Third-Party Service Acquisition: Plan and assist in obtaining services from third-party providers to meet the individual's needs.
Qualifications and Skills
- Education and Experience: Bachelor's degree in human services or equivalent education and experience required; one year of related work experience and knowledge of case management.
- Valid Driver's License: Possess a valid driver's license, registration, and insurance.
- CPR/First Aid Certification: Hold current CPR/First Aid Certification as required by state/program regulations.
- Communication Skills: Demonstrate competence in verbal and written communication skills to effectively interact with individuals served.
- Crisis Management: Ability to handle crisis situations with compassion and professionalism.
