This position is a part of the crisis services team of the Behavioral Health Program. Case Managers in the CAT program provide assessment and recovery focused services for individuals and families who may have mental health needs. As part of the team, Crisis Case Managers participate in clinical staffing, treatment recommendations, information and referral inquiries, crisis intervention, and provide consultation within the agency, to community service partners and the public. The primary function of this position is to provide in-depth assessments, short term case management and appropriate referrals to meet identified needs.
- Complete comprehensive initial assessments that reflect the client and families need for mental health services to include presenting problems, psychosocial history, diagnostic impressions, functional screens, and recommendations for treatment.
- Provide results of the assessment to the client and family as well as treatment alternatives, client’s rights and responsibilities, the anticipated fees for treatment, and the clinic’s grievance procedure.
- Develop a treatment or referral plan in collaboration with the family based on the initial comprehensive assessment to include strengths, treatment goals, measurable outcomes, the frequency and type of service recommended to reach goals.
- Conduct follow up with the family to evaluate progress toward goals, the need for additional assessment or changes in the plan.
- Provide crisis intervention to open families which may include development of a crisis plan and/or collaboration with our crisis system service partners in the community.
- Maintain client’s electronic record in accordance with confidentiality rules and regulatory requirements and standards. Timely completion of documentation is required to include the initial (and subsequent) assessments, progress notes, staffing/supervision meetings, collegial clinical collaboration, treatment plans and reviews, collateral contacts, phone calls/texts/emails, and a closing/discharge summary.
- Monitor need for and complete any documentation/forms required by third party payers to support reimbursement of services provided. Provide accurate, adequate documentation in electronic record to support billing and reimbursement of services rendered.
- Perform short term, case management functions for open families.
- All other duties as assigned.
- Bachelor's degree in a Human Services related field required. Master’s Degree in a Human Services related field preferred for CAT Case Manager, Corrections CM, and for 51 Liaison. A combination of education and experience that provides commensurate knowledge, may be substituted for the Master's Degree.
- Comprehensive and working knowledge of the symptom presentations of all major psychiatric and personality disorders as described in the DSM-V and experience working with these populations.
- Knowledge of or experience with trauma-informed principles and practices is preferred.
- Ability to establish positive connections and develop relationships with clients, their natural supports, and other professionals working within our crisis service system.
- Ability to be sensitive to and respectful of the life experience, individual differences, belief systems and practices, and the motivation for change of each client.
- The capacity to meet each client where they are and instill hope for recovery. Ability to maintain a calm demeanor and make good clinical decisions while adhering to clinical, ethical, and regulatory standards and comfortable seeking consultation when needed.
- Ability to demonstrate appropriate respect for the opinions of others and yet, demonstrate skill in providing opposing opinion if need in advocating for the most clinically appropriate outcome.

- Capacity to uphold the professional code of ethics and HIPAA standards for the protection of personal health information in all service provision and crisis system collaboration efforts.
- Unlimited access to a car, valid driver’s license, and ability to drive or arrange travel in and out-of-county, as requested by supervisor or his/her designee.
- Regular contact with clients, hospital staff, Behavioral Health Program clinicians and case managers, representatives from our crisis services continuum, other SCHHS staff, community service partners, school personnel, and members of the public.
- May occasionally deal with aggressive or violent clients.
- Work is performed in a standard office setting, in homes, and in schools, with occasional need to provide services in other locations. There may be travel to a variety of community and/or regional locations.
- Ability to lift up to 20 pounds and a combination of sitting and standing postures may be required.
- Risk of blood-borne pathogen exposure is considered low for Social Worker, and considered moderate for Crisis Case Manager.
