Key Responsibilities:
- Conduct Psychosocial Assessments: Perform comprehensive assessments to identify the social, emotional, and psychological needs of new residents.
- Develop Care Plans: Collaborate with healthcare professionals to create personalized care plans that address the unique needs of each resident.
- Provide Counseling: Offer individual and group counseling sessions to help residents cope with the challenges of aging and living in a nursing home.
- Advocate for Residents: Act as an advocate for residents' rights and needs, ensuring they receive appropriate care and services.
- Facilitate Communication: Serve as a liaison between residents, their families, and the nursing home staff to ensure clear and effective communication.
- Support Adjustment: Assist residents in adjusting to life in the nursing home, helping them feel comfortable and engaged.
