- Monday to Friday from 8:30 am – 4:30 pm (with hybrid flexibility after a 90-day probationary period).
Job Summary:
We’re looking for a detail-oriented, organized, and communicative individual who thrives on managing multiple projects and people simultaneously. As a Program Manager, you’ll be responsible for providing program and residential management services for a caseload of individuals, supporting them in meeting their personal and developmental goals. You’ll also work closely with a variety of stakeholders, from volunteer providers to medical professionals, to ensure smooth operations and compliance with all applicable state regulations.
Key Responsibilities:
- Oversee and manage a caseload of 10-12 individuals, ensuring program goals are met and services align with state regulations.
- Serve as a back-up Program Manager when necessary to ensure seamless coverage.
- Foster a supportive environment for consumers in line with their Individual Service Agreement (ISA), ensuring all needs are met by the volunteer providers.
- Participate actively in consumer-centered meetings, contributing to goal-setting and providing clear, consistent communication to all parties.
- Collect and maintain detailed documentation, including daily notes, progress reports, and compliance documentation, ensuring all information is accurate and timely.
- Perform monthly site visits to evaluate progress on goals, assess living environments, and address any concerns.
- Collaborate with behaviorists and healthcare professionals to implement behavioral and healthcare plans.
- Ensure timely and appropriate reporting, from incident reports to monthly progress updates.
- Support consumers in medical care, attending key appointments, and troubleshooting any challenges with healthcare providers.
- Ensure compliance with state inspections and implement corrective actions based on findings.
Skills Needed:
- Exceptional organizational skills with the ability to manage multiple priorities at once.
- Strong communication skills to effectively liaise between consumers, providers, and other team members.
- Attention to detail, particularly when it comes to documentation, compliance, and scheduling.
- Proficiency with computer systems and documentation tools.
- The ability to problem-solve and manage emergencies when they arise.
