The Memory Care Coordinator plays a pivotal role in overseeing the operations of the memory care neighborhood within the assisted living community. This leadership position is responsible for ensuring the highest quality of care for residents with memory impairments, including dementia and Alzheimer’s disease. The ideal candidate will be a LPN with a passion for memory care, experience in serving older adults and a desire to learn and grow. Duties
The Memory Care Coordinator will work with the Health and Wellness Director to:
Develop and implement individualized care plans for residents based on their unique needs and preferences
Supervise and train team members in best practices for memory care and medication administration.
Ensure compliance with all regulatory requirements related to memory care services.
Collaborate with families to provide education and support regarding dementia care strategies.
Monitor resident health and well-being, making adjustments to care plans as necessary.
Manage daily operations of the memory care community.
Foster a positive community atmosphere that promotes social engagement and cognitive stimulation for residents.
Conduct regular assessments of resident progress and adjust care strategies accordingly.
