GENERAL SUMMARY:
Responsible for the coordination of services required for an organized multidisciplinary team approach to patient care. Assures quality cost efficient care for the rehab patient population. Develop relationships with organizations and individuals that may refer patients to Schwab Rehabilitation Hospital. Provide on-site assessments at referring hospitals to determine patient appropriateness for rehabilitation. Work with referral sources and Schwab's Admitting Department to facilitate timely admissions. Assist Schwab in achieving {budgeted} inpatient census and {budgeted} outpatient units.
ESSENTIAL FUNCTIONS AND DUTIES:
- Assess the patient/family needs related to medical diagnosis, treatment options, financial resources and psychosocial needs on patients referred to Schwab from the Community as applicable. (outpatients)
- Works collaboratively with interdisciplinary teams, department members and other healthcare provides both internal and external to the organization to improve patient care through effective utilization and monitoring of healthcare resources.
- Assist patient and/or designee in making appropriate referrals depending on patient level of care/needs in the Community.
- Responsible for overseeing the Utilization Management processes for therapeutic treatment plans on assigned patients and assisting in process when necessary/requested.
- Maintains necessary Case Management documentation based on referral source's needs and as applicable on a case-by-case basis.
- Monitors post discharge services as applicable and provide needed referrals.
- Participates actively in team and department meetings. Serves on hospital committees as designated.
- Acts as Patient Advocate in the Outpatient Area, pursuing patient issues that cannot be resolved.
- Communicates with third party payers as indicated for certification or benefit determination and referral development/enhancement.
MINIMUM EDUCATION:
Masters degree in social work.
MINIMUM WORK EXPERIENCE:
Two (2) years experienced preferred, preferably in rehabilitation setting and/or discharge planning.
KNOWLEDGE, SKILLS AND ABILITIES:
Excellent written and verbal communication skills.
REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:
Licensure to practice in the State of Illinois.
