Job Overview
WV Birth to Three service coordinators provide case managment support and advocacy to families with infants and toddlers with delays and disabilities in WV
Duties
- Manage and oversee service delivery for families that qualify for the WV Birth to Three System.
- Develop and implement individualized service plans in collaboration with families and teams.
- Maintain strong relationships with families and external service providers to facilitate effective communication and service coordination.
- Monitor program effectiveness through regular assessments and adjustments to services as needed.
- Maintain accurate documentation of client progress, service plans, and administrative records in compliance with regulatory standards.
Requirements
- Experience working with individuals with developmental disabilities is preferred.
- Background in social work or behavioral health is a plus.
- Strong organization, communication and technology skills and with the ability to lead a team effectively.
- Excellent relationship management skills to foster positive interactions with families, WV Birth to Three team members and community partners.
- Proficiency in program development and management practices.
- Strong organizational skills with attention to detail for administrative tasks.
- Ability to work collaboratively within a multidisciplinary team environment.
