• Weekly Gross: $4000 based on 40 hours

Job Summary:

Partners in Home Care Inc. is seeking a highly motivated and dynamic leader to direct our Hospice & Palliative Care program. The Director will oversee operations, strategic planning, fiscal management, employee and patient safety, quality of care, service delivery, business development, and community relations. This position reports directly to the CEO.

Essential Duties and Responsibilities:

Financial Management:

Responsible for quality and financial performance outcomes.

Develops and oversees budgets for all Hospice and Palliative clinical operations.

Compliance:

Ensures compliance with applicable laws, regulations, and accreditation standards.

Maintains knowledge of CMS regulations and Conditions of Participation for Hospice and Palliative programs.

Program Management:

Develops and monitors departmental KPIs.

Establishes patient care standards using evidence-based practices.

Oversees new program initiatives and collaborates with internal and external partners.

Talent Management:

Evaluates program resources and staffing.

Establishes performance standards and guides’ staff.

Develops an environment that supports excellence and positive change.

Leadership:

Maintains clear communication across programs.

Partners with internal teams for program growth and market share improvement.

Participates in collective bargaining activities.

Qualifications:

Proven leadership experience in Hospice and Palliative Care.

Strong financial and compliance management skills.

Excellent communication and team management abilities.
