Job Title : Licensed Practical Nurse (Full Time)
Job Specialty : Long-Term Care (LTC) LPN

Shift : 8 Hours, Day / Evening
Guaranteed Hours : 40 hours per week
Experience : Minimum one year of experience as LPN in LTC or CNA capacity
License : LPN Licensure
Certifications : Basic Life Support (BLS)

Job Description :
- Provide direct and indirect resident care under licensed nursing personnel's guidance, within the defined scope of practice.
- Demonstrate knowledge and skills to provide age-appropriate care for adult and geriatric patients.
- Meet residents' Activities of Daily Living (ADL) needs in a safe and comfortable environment as outlined in the plan of care, including hygiene, grooming, dressing, incontinence / catheter care, elimination / toileting needs, turning / positioning, and skin care.
- Maintain residents' environment and safety, including answering call lights and alarms promptly, maintaining cleanliness, behavior and fall prevention, and monitoring equipment for malfunction.
- Ensure residents' nutritional needs are met, honoring preferences and choices, including meal delivery, tray preparation, and use of adaptive equipment.
- Address residents' rehabilitative needs as outlined, including range of motion, ambulation, transfers, body alignment, and assistive devices.
- Report residents' rights, refusals, and concerns to the charge nurse promptly, and identify and report changes in residents' physical, emotional, and psychosocial status.
- Perform tasks involving potential contact with blood, body fluids, or tissues, taking necessary precautions for occupational exposure to bloodborne pathogens.
