Job Title : Licensed Practical Nurse (LPN) - Part Time

Job Description :
- Perform direct and indirect resident care within the scope of practice as defined by certification under the direction of licensed nursing personnel.
- Meet resident's Activities of Daily Living (ADL) needs as outlined in the care plan, such as hygiene, grooming, dressing, incontinence care, elimination, turning, positioning, and skin care. Complete documentation per protocol.
- Maintain resident's environment and safety in line with the care plan, respond to call lights and alarms promptly, ensure safety mats and low beds are used, cleanliness of unit is maintained, and practice behavior and fall prevention. Complete documentation per protocol.
- Meet resident's nutritional needs as outlined in the care plan; deliver meals, prepare trays, use adaptive equipment, and assist with feeding. Honor resident preferences and choices. Complete documentation per protocol.
- Meet resident's rehabilitative needs as outlined in the care plan, including range of motion, ambulation, transfers, proper body alignment, and the use of assistive devices. Complete documentation per protocol.
- Report resident's rights, refusals, and concerns to charge or supervising nurse in a timely manner in accordance with policies and procedures.
- Identify and report changes in resident physical, emotional, and psychosocial status to charge nurse promptly.
- Anticipate occupational exposure to bloodborne pathogens; required tasks may involve potential contact with blood, body fluids, or tissues.
