Position Summary:
The Clinical Social Worker will use a person-centered approach to work with individuals who have complex medical and mental health needs, both in short or long-term capacities. Depending on the client and their unique needs, the social worker may work collaboratively with the client and outside providers to conduct crisis intervention, refer and access needed services, conduct psychotherapy and lead/coordinate support groups. The employee will primarily work within the parameters of the Connect Program within the Behavioral Health Outreach For Aging Populations (BHOAP).
Job Responsibilities and Performance Standards:
· Work directly with older adults who are experiencing emotional/mental health challenges to assess their needs, help stabilize them and link them with short-term and long-term therapeutic services, as needed.
· Maintain caseload of clients with mental health issues who require short-term and long-term case management and/or psychotherapy in order to remain safely in the community.
· Provide crisis intervention services as needed.
· Become knowledgeable of local resources and programs, both mental health-related and otherwise, and make referrals as needed.
· Provide advocacy, support, and assistance in areas commonly affected by mental health diagnoses such as: housing, hoarding, substance abuse, medication compliance, and communication with providers.
· Conduct outreach to local providers about SCES’s in-home behavioral health progams.
· Serve as group leader for psychotherapy, support or educational groups.
· Maintain timely and appropriate case documentation according to state regulations, agency standards, and BHOAP grant requirements.
· Participate in the local Mental Health and Aging Coalition task force and other mental health provider groups, as relevant.
· Perform other duties as assigned by supervisor.
· Comply with HIPAA guidelines.

Qualifications:
· Licensure or eligibility for licensure: LCSW, LICSW, LMHC or LMFT
· Two plus years of experience working with elders strongly preferred
· Ability to effectively handle crisis or emergency situations
· Experience working with individuals and families around mental illness or substance abuse issues
· Fluency in a second language preferred
· Must be knowledgeable and comfortable using Adobe and MS Word, Excel, Powerpoint, Sharepoint, and other MS 365 platforms. Experience with Wellsky strongly preferred.

· Physical surroundings are comfortable with minimal exposure to injury or hazards.
Social/Psychological Conditions:
· Occasional stress due to periodic or cyclical workload pressures and deadlines. Some interruptions involved.
Physical Effort:
· Frequently sits, stands, walks, bends, reaches, and stoops throughout the day.
· Frequently lifts, pulls, pushes, and carries up to 20 lbs.
· Periodic eye strain and light ear strain.

Pay: $65,000.00 - $68,000.00 per year
