Requirements/Qualifications: A baccalaureate degree plus two years of professional level experience.
Function of work: This position serves as a Disease Intervention Specialist at the Regional STD/HIV Program. The core functions of this position are to: demonstrate sound practices and performance in disease investigations and case management, work closely with the regional manager or supervisor in HIV prevention, and act in an advanced lead responsibility in community education/outreach regarding HIV and other STD.
Examples of Work:
- Conducts interviews of persons newly diagnosed or suspected of having a HIV and/or STDs in order to elicit names and locating information of person exposed.
- Identifies, locates, and persuades cases, contacts, suspects, and susceptible to report to medical facilities for examination and follow-up.
- Recommends and urges physicians and other health care providers to use recommended therapy and appropriate laboratory tests for potential or confirmed cases of communicable disease.
- Conducts case management assessments to ascertain further possible potential for spread of disease and initiates appropriate new action.
- Prepares and maintains confidential records; collects, documents, and verifies pertinent medical data from patients, physicians, and medical facilities.
- Motivates patients and educates the general public on behavior modification, risk reduction, and prevention of communicable disease.
- Obtains blood and exudates specimens; performs limited physical examinations and clinical tests to determine the existence of infections or exposure to disease.
- Responsible for educational presentations to various community groups regarding HIV and other STDs.
35%- Disease Intervention Specialist
- Carry-out and coordinate investigative and case management activities and screenings in various community settings and is identified as a lead/liaison to establish or maintain collaborative relationships with improved communication and partnerships that affect disease outcomes in clients and populations.
- Provide internal and external STD and HIV presentations or trainings to increase awareness of disease, trends, and program services.
35%-Disease Follow-up and Case Management
- Perform quality, demonstrated field and case management activities in various settings to include: pre-field searches through available resources, systems and files; phone calls to medical providers; field investigations and pouch management; disease intervention interviews; partner/cluster elicitation, notification and referrals; case analysis and management; risk assurance that persons with positive lab finding for HIV, and their exposed partners or other at risk individuals, are referred for services such as treatment and case management.
- Utilize available data system(s) and maintain data security and confidentiality in accordance with state and programmatic laws, policies, and procedures.
- Strive to meet or achieve programmatic timelines and documentation on disease related electronic records.
10%- Community Education, Screenings and Partnerships
- Establish or maintain collaborative relationships with community groups; and assist with targeted outreach screenings to identify populations impacted by, or at risk of HIV/AIDS.
- Use proper laboratory procedures to obtain blood and oral specimens-with approved training, observed practice and certification.
15%-Communication
- Communicate effectively in writing and verbal with all supervisors, senior level management staff, co-workers, and medical personnel.
- Maintain positive working relationships other staff members.
- Prepare written reports.
- Follow state and programmatic guidelines.
5%- Other Duties Assigned
- Attend seminars, workshops, in-service trainings, required meetings, and perform other related duties as required.
