What are the responsibilities and job description for the SOCIAL WORKER (BSW) position at Southeastern Regional Medical Center?
POSITION SUMMARY: Assesses, plans and coordinates the discharge planning process for patients, as well as arranging post-acute care services for patients in a timely healthcare delivery. Additional responsibilities include serving as a patient advocate, utilizing resources efficiently to improve patient outcomes, and follow through as needed with post hospital care needs to ensure continuity of care.
EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE :
Minimum Required: Bachelor's Degree in Social Work, Sociology or Psychology from an accredited institution. Working knowledge and understanding of the social work and discharge planning processes gained through previous experience or an approved internship.
Preferred: Master's Degree in Social Work, with 2 years of experience as a Medical Social Worker or with Discharge Planning.
ADDITIONAL SPECIALIZED KNOWLEDGE/SKILLS : Incumbent must demonstrate an ability to apply age-appropriate principles of communication and social
work practice relevant to neonatal, infant, pediatric, adolescent, adult and geriatric patients. Incumbent must demonstrate competence in the use of equipment and
a pplication of procedures before being allowed to perform independently.
