Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this pivotal role, you will be responsible for coordinating patient care, ensuring that patients receive the appropriate services and support throughout their healthcare journey. You will work closely with patients, families, and healthcare providers to facilitate effective care plans, particularly in areas such as infectious disease care and medical-surgical needs. Your expertise in medical terminology and patient service will be essential in delivering high-quality care within our Level I Trauma Center.
Duties
- Assess patient needs and develop individualized care plans to ensure optimal patient outcomes.
- Collaborate with interdisciplinary teams to coordinate services and resources for patients.
- Monitor patient progress and adjust care plans as necessary, utilizing effective utilization management strategies.
- Provide education and support to patients and their families regarding treatment options and health management.
- Document all patient interactions, assessments, and care plans accurately in the electronic health record system.
- Advocate for patients within the healthcare system to ensure they receive appropriate services.
- Participate in quality improvement initiatives to enhance patient care processes.
- Stay current with medical terminology and best practices in nursing and case management.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Proven experience in patient care, preferably within an ER or trauma setting.
- Strong knowledge of medical terminology and utilization management principles.
- Experience with infectious disease care is a plus.
- Excellent communication skills with a focus on patient service and advocacy.
- Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
- Familiarity with electronic health records (EHR) systems is preferred.
- Commitment to ongoing professional development and adherence to nursing standards of practice.
REQUIRED:
Previous acute inpatient case management experience, 3-5 years case management experience, strong computer skills, Epic EMR experience, BLS certification from AHA.*** Candidates with utilization management, therapy, or telephonic case management experience are NOT appropriate for this position. On site training will be only one week – candidates will need to learn quickly and hit the ground running. Typical Case Ratio is 1:21 – 25, will not be meeting with every patient – this is decided based on individual risk factors.

Pay: $45.00 - $50.00 per hour
