What are the responsibilities and job description for the Human Services Planner/Evaluator I position at State of North Carolina?
The primary purpose of this position is to Review and process referrals, process vacancy notices from properties and available subsidies to match with people with disabilities in need of housing, including but not limited to Transition to Community Living Initiative participants, Money Follows the Person participants, homeless individuals with disabilities, and the general population of individuals with disabilities. Manage the Wait/Interest List for available DHHS Housing Resources. Update Targeted Unit Database with data about available properties, property contact information, referral agencies, property waitlists, application process and tenancy crisis. Participate in Housing Team Meetings to maximize Effectiveness of DHHS Housing Programs, identify areas for program growth, suggest ideas for improvement and collaborate with team members to determine if policy changes are needed including (staffing team calls, Peer to Peer technical assistance, strategizing program changes related to new initiatives, professional development, and relevant workshops and other training activities. Handle phone calls, emails and other communications to provide program guidance and technical assistance to stakeholders on housing resources and Fair Housing Review.

- Experience of housing and homeless programs.
- Experience of permanent supportive housing for adults with disabilities.
- Experience with public funding streams for affordable housing.
- Experiencee with Low Income Housing Tax Credits, Section 8, NC Housing Trust Fund, HOME, HUD 811.
- Analytical skills and ability to analyze and interpret data.
