Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will have experience in outpatient settings, hospitals, clinics, or home health environments. The Physical Therapist will work collaboratively with other healthcare professionals to ensure comprehensive patient care.
Responsibilities
- Evaluate patients' physical abilities and limitations through assessments and tests.
- Develop and implement personalized treatment plans based on individual patient needs and goals.
- Utilize knowledge of anatomy and physiology to guide therapeutic interventions.
- Educate patients and their families about treatment plans, exercises, and self-care techniques.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Collaborate with multidisciplinary teams to coordinate discharge planning and follow-up care.
- Maintain accurate documentation of patient evaluations, treatments, progress notes, and discharge summaries.
- Stay updated on best practices in physical therapy and participate in professional development opportunities.
Qualifications
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Valid state license to practice as a Physical Therapist.
- Experience in outpatient therapy, hospital settings, or home health preferred; pediatric experience is a plus.
- Strong knowledge of anatomy, physiology, and rehabilitation techniques.
- Excellent communication skills for effective interaction with patients, families, and healthcare team members.
- Ability to develop comprehensive care plans tailored to individual patient needs.
