Essential Duties and Responsibilities:
-
Designing processes and tools to reach out to patient populations overdue for preventative health services via recall lists, waitlist and other resources that are available.

Work with patients in person, via telephone, and other communication methods on improving their overall wellness and schedule necessary appointments.

Utilize EHR (Epic) and other data systems to retrieve and/or input patient demographics and health data.

Schedules and cancels appointments according to preset scheduling guidelines.

Document appropriate communication in patient charts after each phone encounters.

Offers the patient guidance and education related to their specific needs.

Connects patients to resources within STRIDE and the community to address identified health concerns.

Provides timely documentation and required reports for all population health management activities

Contributes to population health efforts including targeted outreach and scheduling of patients based on specific criteria such as the presence of a chronic condition, wellness visit due, preventative health needs, recent emergency department or hospital visit, or at provider/organization request.

Follows departmental standard workflows.

Supports quality improvement activities including those informed by external partners, payors, accreditors, and regulators as assigned.

Demonstrates professional behavior with the public, patients, and peers at all times

Registers, check-in, and checks-out patients for all services
-
Maintains charts, and ensures Health Insurance Portability and Accountability Act (HIPPA) and OSHA compliance while demonstrating understanding of regulation

Collects and uploads all necessary documentation and consents

Promptly and accurately updates patient records including financial and demographic information
-
Promptly coordinates scheduling of new patient referrals, gathers necessary information, and schedules appointments accordingly
-
Collects copayment and account balances at time of service

Acts as liaison between patients and Billing Department regarding charges, payments, and insurance
-
Contacts patients to follow up on no-shows and reschedule appointment

Ensures all schedules are filled and maintained
-
Completes all other duties as assigned

Qualifications
Job Qualifications:
A successful candidate will possess the following education or formal training:

Excellent customer relation skills and ability to communicate professionally and effectively with and patients and practice staff.

Ability to work remotely in a quiet, designated workspace free of distraction and commute to Stride clinics as assigned adhering to the Stride Remote Work Policy.

Knowledge of healthcare IT, patient centered medical home concepts.

Education or Formal Training:
-
Preferred: Associate degree or equivalent in healthcare administration
-
Will consider: High school diploma or GED
-
Spanish language proficiency preferred
Experience: 1 years related experience or customer service experience preferred.
Material and equipment directly used: Computer, EHR (Epic) printer, multi-line telephone systems, fax machine, copy machine, calculator, Microsoft Office products including Outlook, Excel, TEAMS, and internet.
