Individual Service Coordinator
Position Statement:
The Individual Service Coordinator is a Qualified Intellectual Disabilities Professional who has the responsibility to complete Pre-admission Screening and provide linkage to services for individuals residing in the community. The Individual Service Coordinator is also responsible for the oversight of srvices for individual with developmental disabilities residing in community settings funded by the Department of Human Services/Office of Developmental Disabilities.
Duties:
- Coordinate and oversee services for individuals with developmental disabilities
- Develop and implement individualized service plans
- Conduct assessments to determine client needs and goals
- Provide ongoing support and guidance to clients and their families
- Collaborate with community resources to ensure access to necessary services
- Monitor and evaluate client progress and adjust service plans as needed
- Maintain accurate documentation and records of client interactions
Skills:
- Strong background in social work or related field
- At least one year of experience working with individuals with developmental disabilities
- Administrative skills, including record keeping and data management
- Knowledge of behavioral health principles and strategies
- Excellent relationship management skills, including communication and collaboration
- Strong organizational and time management skills
Qualifications:
This position requires a Bachelor's degree in a Human Services field and a minimum of one year of experience working directly with individuals with Intellectual Disabilities or a related condition.
