Qualifications
1. Community Service Coordinator/Case Manager experience highly required
2. Knowledge of Baltimore, MD area (Required)
3. Excellent working relationship with Service Coordinators
4. The ideal candidate must be outgoing, goal-oriented, and works well under pressure
5. The candidate selected must have their own reliable transportation and works well under minimal supervision
6. The candidate selected must be able to use their personal phone and laptop (computer) for communication and correspondence
7. Driver’s License
8. Skilled in establishing rapport
9. Excellent written and oral communication skills
10. Creative mindset: ability to think outside the box
11. Ability to work independently
12. Cumulative outreach experience, and/or building client relationships
13. Computer experience with Office 365/One Drive and Microsoft Office products
14. Basic knowledge of outreach tactics
15. The candidate selected must be able to work in the field, as this is a field-based position
16. Willingness to travel to any part of Maryland to seek out potential clients when necessary
17. Bachelor’s degree in social work, communications, public relations, or a related field (or equivalent experience)
18. Minimum of 2 years of experience in community outreach
19. Indept knowledge of Developmental Disabilities Administration (DDA) services and programs in Maryland
Responsibilities
1. Primary responsibilities include outreach and referral activities in surrounding communities, including but not limited to: doctors’ offices, healthcare facilities, nursing homes, special hospitals, and individual practices within a health system
2. Provide input on collateral consistent with the brand including but not limited to, brochures, flyers, posters, email newsletters, and banners to support lead generation efforts
3. Visit potential leads on-site to distribute marketing material and discuss agency services
4. The Community Outreach Specialist will play a critical role in our agency's efforts to recruit clients for our various programs (Adult Residential Services, Personal Support Services, Community Development Services, Nursing Support Services, and Supported Living Services)
5. Develop and implement outreach strategies to engage potential clients for the agency’s programs
6. Establishes direct relationships with clients in the community and assists them with self-referral into services.
7. Build and maintain relationships with community organizations, schools, and other referral sources
8. Conduct outreach activities such as presentations, workshops, and community events
9. Coordinate client intake and enrollment processes
10. Maintain accurate and up-to-date client records and documentation
11. Provide support to clients and families throughout the enrollment process
12. Monitor and track program enrollment and outcomes
13. Participate in team meetings and training sessions
14. Networking with potential referral agencies with the goal of generating interest in referring adults to our programs within the Baltimore area. (And other areas, as needed).
15. Represent the agency at community events, health fairs, and other public forums.
16. Partnership Development: Identify, cultivate, and maintain relationships with local organizations, schools, healthcare providers, and other potential partners to expand referral networks and collaborative opportunities.
17. Client Outreach: Develop and implement strategies to reach out to potential clients and encourage them to utilize the services offered by our agency. Follow up with clients and referrals to ensure engagement and satisfaction.
