Position Overview:
Sun River Health is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our team at the Bronx Blvd location in Bronx, NY. This full-time position provides an exciting opportunity to work within an interdisciplinary team, delivering integrated care to patients facing various health and social challenges, including chronic illnesses, HIV, aging, and domestic violence. The LMSW will offer crisis intervention, case management, biopsychosocial assessments, and counseling services, all while maintaining a patient-centered approach aligned with Sun River Health's values.
Key Responsibilities:
- Comprehensive Assessments & Care:
- Conduct biopsychosocial assessments for new patients referred by primary care providers or other service providers to address both medical and social/emotional concerns.
- Perform mental status exams and collaborate with the healthcare team, including psychiatrists and primary care providers, for treatment planning and ongoing care.
- Direct Counseling & Support:
- Provide individual, family, and group counseling to patients and their families, particularly those affected by chronic illness, HIV, domestic violence, and other psychosocial issues.
- Offer crisis intervention and emergency evaluations to support patients in immediate need.
- Case Management:
- Coordinate case management services to ensure patients receive the appropriate care, support, and referrals to other specialists or community resources as needed.
- Collaborate with external providers for additional psychiatric evaluations or more intensive services.
- Collaboration & Teamwork:
- Work closely with the medical, behavioral health, and HIV care teams to develop and implement holistic treatment plans for patients.
- Participate in interdisciplinary case conferences and contribute to integrated care strategies, including group therapy sessions.
- Documentation & Compliance:
- Ensure all clinical documentation is completed accurately, on time, and in compliance with Sun River Health's standards and regulatory requirements.
- Maintain up-to-date case records and ensure compliance with grant and funding requirements.
- Prevention & Education:
- Develop and implement prevention strategies for individuals at risk for HIV, chronic conditions, or other negative health/social outcomes.
- Support the community by facilitating access to resources and educating patients on self-management of health.
Qualifications:
- Education: Master’s degree in social work (MSW) from an accredited program.
- Licensure: New York State Licensed Master Social Worker (LMSW) required.
- Certifications: CPR/BLS certification required.
- Experience:
- A minimum of 1-2 years of experience in social work, particularly in a healthcare setting, is preferred.
- Experience working with underserved populations, those living with HIV, or individuals experiencing chronic illness, domestic violence, and aging is a plus.
Salary Range: $72,982.00 - $76,895.00 per year (based on licensure and experience)
