SSVF Shallow Subsidy Case Manager

Schedule (Days and Hours):

Monday – Friday; employee may choose working hours of 8:00 am – 4:00 pm or 9:00 -5:00pm

 

Reporting Type (In-person, Hybrid, or Remote):

In-person, with possibility of one remote day per week after 90 days

 

Employment Type (Non-exempt / Exempt) AND (Full-Time/Part-Time/ On-Call):

Full-time; Non-exempt

 

Salary Range (Hourly if Non-Exempt, Annual if Exempt):

$31.03

 

Summary:

The Shallow Subsidy (SS) Case Manager will work in collaboration with other SSVF staff and Program Management to assess and determine eligibility of referred veteran clients to enroll and participate in the SSVF Shallow Subsidy program. The SS Case Manager will be assigned veteran households from both the San Francisco and Oakland offices, providing case management services and subsidy rental assistance for up to 24 months. Utilizing evidence-based best practices, the SS Case Manager will work closely with veteran households to achieve and maintain housing stability for the duration of their enrollment in the Shallow Subsidy program and beyond. The SS Case Manager will utilize community-based partnerships with Supportive Housing Programs, VA VASH Voucher Program, and market-rate rental landlords to establish and expand housing resources and eviction prevention remedies. The SS Case manager will use these resources, in conjunction with SSVF program team and Drop-In program staff, as needed, to assist veterans in finding appropriate permanent housing and/or preventing eviction. This position will be based primarily at the Oakland Office but will require the ability to travel to and work at the San Francisco Office occasionally.

 

Responsibilities:

  • Perform intakes, assessments, and eligibility determination
  • Work with the veteran household to develop individualized housing stability plans
  • Maintain monthly contact with clients and conduct quarterly financial check-ins to monitor and track participants’ progress towards the end-goal of achieving financial stability and long-term sustainable housing
  • Make internal and external referrals to additional supportive services
  • Maintain a working understanding of the readjustment problems of veterans, Post Traumatic Stress Disorder, Traumatic Brain Injury, alcohol and drug dependency, and other mental health issues
  • Adhere to laws regarding confidentiality and reporting requirements; maintain a knowledge of current HIPAA policies and procedures
  • Maintain timely and appropriate documentation including progress notes, housing stability plans, aftercare plans, and discharge summaries
  • HMIS/ECM/data record management
  • Ability to conduct housing inspections as needed in San Francisco, Alameda, San Mateo, Contra Costa, and Marin Counties
  • Assist veteran households with money management and budgeting as needed
  • Assist veterans with establishing disability income and other benefits as needed
  • Participate in weekly client case conference meetings and weekly supervision with the SSVF Shallow Subsidy Program Manager, and other meetings as required
  • Perform other related duties as required by the SSVF management staff
  • Maintain an ability to carry out the essential duties of the position within what can be considered reasonable accommodation

Desired Qualifications:

Experience:

  • Minimum of one (1) years’ experience working with “hard to serve” individuals, e.g., homeless, substance abusers, and persons with mental illness in areas of housing stabilization, placement, and retention
  • Strong data entry skills
  • Basic knowledge of mental health symptoms and diagnoses, psychosocial stressors, and chemical dependency issues
  • Crisis intervention skills, including de-escalation and conflict resolution skills
  • Good written and verbal communication skills
  • Ability to recognize relapse behavior
  • Ability to work both independently and as a member of a team
  • Translatable military experience and skills may be alternatively substituted

 

Education:

  • BA degree preferred in Behavioral Sciences; relevant experience may be substituted
