Certified Diabetes Care and Education Specialist (CDCES) Registered Nurse
Job Description
Outpatient services are provided IN PERSON at our 3 locations throughout sunny Central Oregon. Local travel will be required.
Status: Part-Time

Responsibilities
Patient Care and Education
- Using evidence-based weight-inclusive practices, conducts assessments, interventions, DSME, and follow-up in a patient-centered manner for patients referred by area providers.
- Obtain and maintain certifications in insulin pump management. Educate and support patients with utilizing technology for diabetes management.
- Collaborates with a team of dietitians.
- Develop, conduct, maintain, and provide support for diabetes and nutrition group education programs.
- Utilize motivational interviewing (MI) skills to support patients in a patient-centered manner.
Administrative
- Participate in collaboration of care and marketing to area providers.
- Utilize technology effectively and efficiently for scheduling, communications, and record keeping.
- Maintains accurate records and completes documentation in a timely manner.
- Provide oversight and record-keeping, compliance for ADCES accredited diabetes program.
Qualifications
Required
- State of Oregon Licensed Registered Nurse (active)
- CDCES credential
- Belief in and adherence to Health at Every Size® and Intuitive Eating philosophies
Schedule:
- Monday to Friday
- Weekends required--8-12 Saturdays per year

Synergy’s diabetes program is accredited by the Association of Diabetes Care and Education Specialists (ADCES) and provides individual and group educational support to patients of all ages.
Our diabetes programming and medical nutrition therapy services are offered at 3 locations throughout Central Oregon.
Clientele served include people with diabetes of all ages and types, and medical nutrition therapy clients with varied conditions. Services are provided to individuals and groups.
