Key Responsibilities:
Provide comprehensive case management services to patients, ensuring effective care coordination and discharge planning.
Utilize excellent verbal and written communication skills to collaborate with healthcare teams, patients, and families.
Deliver exceptional customer service while interacting with internal and external stakeholders at all levels.
Apply strong problem-solving, analytical, and decision-making skills to support patient care.
Leverage expertise in community health, rehabilitative medicine, home health care, and managed care to connect patients with appropriate resources.
Demonstrate knowledge of Medicare and Medicaid regulations, case management principles, and preventive service guidelines.
Advocate for patients while addressing social determinants of health that impact overall well-being.
Manage multiple complex patient needs efficiently while maintaining strong organizational and planning skills.
Ensure compliance with medical ethics and legal implications related to case management.

Required Qualifications:
Nursing Degree or Master of Social Work (MSW)
Active, unrestricted Registered Nurse (RN) license or Licensed Master Social Worker (LMSW) license in the State of Michigan
Previous acute inpatient case management experience (3-5 years required)
Strong computer proficiency, including experience with Epic EMR
BLS certification from the American Heart Association
Ability to learn quickly and adapt in a fast-paced setting with limited onsite training
Desired Qualifications:
Experience in discharge planning, rehabilitative medicine, or community health
Familiarity with behavior change theory and clinical practice guidelines
Understanding of social determinants of health and their impact on patient care
Strong ability to facilitate access to community resources for diverse populations
Contract Details:
Duration: 13-week contract
Case Ratio: Approximately 1:21-25 (not all patients require direct meetings; case selection is risk-based)
Onsite Training: One week; candidates must be self-sufficient and able to integrate quickly
Potential for Direct Hire: Candidates interested in transitioning to a permanent role must hold a Bachelor’s degree
