Job Title: Registered Nurse (RN) - Case Manager
We are seeking a compassionate and experienced Registered Nurse (RN) to join our Case Management team in a hospital setting. As an RN Case Manager, you will be responsible for coordinating patient care, ensuring that patients receive appropriate and efficient services, and helping to streamline the healthcare process. You will work closely with a multidisciplinary team to manage the continuum of care from admission to discharge, advocating for patients while ensuring optimal outcomes.
Location: Hospital Setting
Key Responsibilities:
Coordinate patient care by working with physicians, nurses, social workers, and other healthcare professionals.
Assess patient needs, develop individualized care plans, and ensure patients have access to appropriate resources.
Monitor patient progress, intervene as necessary, and facilitate timely discharge planning.
Advocate for patients and families, addressing any concerns or barriers to care.
Ensure patients are discharged with the proper follow-up care and support.
Educate patients and families on healthcare options, resources, and discharge instructions.
Review and manage patient cases to ensure appropriate use of hospital resources and adherence to insurance requirements.
Document all case management activities and communications accurately in the patient’s medical record.
Collaborate with insurance providers to ensure coverage for treatments and services.
Other duties as assigned.
Qualifications:
Active Registered Nurse (RN) license.
Experience in case management or hospital-based nursing preferred.
Strong knowledge of clinical care pathways, discharge planning, and healthcare resources.
Excellent critical thinking, problem-solving, and organizational skills.
Strong communication skills and the ability to work well with patients, families, and healthcare teams.
Ability to manage multiple cases effectively while ensuring quality care.
Shift: Monday–Friday, 8:30 AM - 5:00 PM (OR three 12-hour shifts per week, 11:00 AM - 11:30 PM)
Type: full-time contract employment 13-14 weeks with possibility to extend
