What are the responsibilities and job description for the Outreach Worker (479) position at TESTING KINGSVIEW TESTING?
This position is an integral part of a treatment team.
The Outreach Worker I will work with a high level of independence, assists in service delivery program to homeless clients in the rural areas.
The Outreach worker will engage clients in the field about services and supports available to them.
Enrolling and providing case management and outreach services as needed to clients.
As part of a service delivery team, provide services to mental health, substance abuse and dual diagnosis consumers.
Maintain clear, accurate and concise consumer records, in a timely manner.
Assist with program design and implementation.
Provide professional linkage / consultation with peers, professionals of other disciplines and outside entities.
Perform other duties as assigned.
Perform daily outreach, engagement and linkage to homeless individuals.
Effectively document services provided into HMIS Database.
