Requirements:
- Minimum of 1-2 years medical office experience
- Computer skills and data entry experience
- Goal-oriented and excellent communication skills
- Good command of MS Office
- Medical, home health or hospice office experience required
Job Duties:
- Conduct initial screenings and intake assessments of new clients
- Gather essential client information such as medical history, insurance information, and contact details
- Coordinate with other members of the team to ensure proper care settings for each client
- Communicate with clients and their families regarding the intake process and any necessary documentation or information
- Enter client data into electronic medical records system and update as necessary
- Ensure compliance with HIPAA regulations and maintain confidentiality at all times
- Monitor and track intake process and provide regular updates to team members
- Collaborate with other departments to ensure the smooth flow of information and coordination of care
- Participate in continuing education and training to maintain knowledge of best practices and regulatory requirements.
- Scheduling of clinicians in the field as needed (upon training)
