Hours : 8 : 00 am to 5 : 00 pm

Key Responsibilities

Provide therapeutic and case management services to adults with psychiatric disabilities, improving their functioning in the community.

Conduct outreach and engage potential clients in jails, hospitals, and street settings.

Perform comprehensive assessments to evaluate diagnoses, levels of functioning, and support networks.

Assist clients in setting goals and developing Coordinated Care Plans, ensuring annual updates.

Ensure clients apply for and receive necessary benefits, including SSI, SSDI, Medicaid, and Medicare.

Facilitate access to local resources, such as medical care, housing, rehabilitation programs, and social services.

Engage with clients who have not maintained contact, ensuring they remain connected to services.

Deliver therapeutic interventions, including individual, group, and crisis counseling.

Maintain accurate documentation in the Electronic Health Record (EHR) system.

Meet weekly, monthly, and quarterly billing targets.

Collaborate with team members, participate in 24-hour on-call coverage, and assist with daily operations of related programs.

Qualifications & Requirements :

High School Diploma required; Associate’s degree preferred in human services or a related field, or equivalent life experience.

Ability to provide culturally competent care.

Experience in crisis intervention, education, case management, advocacy, and connecting clients to community resources.

Knowledge of substance use and mental health treatment practices.

Strong relationship-building skills with individuals at risk for substance abuse.

Ability to collaborate with other staff and social service providers.

Ability to work with agencies and organizations to connect clients to services.

Facilitation skills for parenting groups.

Certificates, Licenses & Registrations : Certified Chemical Dependency Assistant (CDCA) required.

Work Environment & Physical Requirements

This position requires regular use of hands, fingers, and the ability to communicate effectively. The Care Coordinator will frequently be required to walk, balance, stoop, kneel, and drive. Occasionally lifting up to 15 pounds is necessary. The work may include driving to various locations and providing transportation for clients.
