Reports To: Administrator
FLSA Status: Exempt
This is a salaried position with weekend on-call coverage rotation.
SUMMARY
The Admissions Liaison for the skilled nursing facility is responsible for the overall operation of the admissions process. The Admissions Liaison is responsible for establishing and maintaining open, effective communication between facility and referring publics, consumers, physicians and third party payers. This individual must be able to maintain a non-adversarial relationship with these referrers. The Admissions Liaison provides maximum flexibility in coordinating the payer’s needs with patient outcomes. This individual must be able to communicate effectively with the treatment team, as well as influence the treatment approach when needed. Market place feedback will be systematically provided to management and the treatment team, and utilized in the development of new or expanded services.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following; Other duties may be assigned.
Identifies and maintains relationships with hospital discharge planners. Notifies of open beds. Fosters positive trust relationships with insurance representatives, referrers and physicians. This position requests Liaison to possess admission cell phone as per an agreed upon schedule with Administrator.
Receives referral from physician, hospital of insurance based case manager on all potential admissions
Conducts telephone and faxed copy assessment to ensure appropriate clinical admission. May conduct on site evaluation if necessary.
Ensures initial and ongoing fiscal appropriateness of patient admissions to the transitional care unit.
Determines patient Medicare, Managed Care and third party insurance coverage for potential and newly admitted patients.
Monitors patient progress and communicates information between patient, referrer, family, insurance company and treatment team.
Ensures development of discharge plan early in treatment program with realistic discharge goals and destination.
Provides assertive consultation and input regarding program development and enhancements.
Obtains preadmission authorization for services from managed care.
Discusses Medicare/Medicaid with families, etc. and makes appropriate referrals to Business office,
Determines patient medical eligibility, qualifying diagnosis, and determines Medicare/Managed Care eligibility based on skilled services provided.
Responds to inquiries from hospital discharge planners related to possible admissions. Reviews assessment for appropriate unit placement within the continuum.
Facilitates the clinical needs of admitted patient by working with the interdisciplinary units.
Maintains resident privacy, confidentiality and adheres to corporate compliance guidelines
Follows established safety and infection control policies and procedures and participates in all required in service training regarding fire safety, disaster preparedness, infection control safety regulations including body mechanics and ergonomics.
Attends and participates in workshops, seminars, etc., to keep abreast of current changes in the long-term care field and maintains appropriate licensure.
Organizes small diversional group activity on unit using available activities to provide increased socialization, person-centered care and enjoyment for residents
Actively participates and supports Quality Assurance Performance Initiatives and cultural changes through Performance Improvement Project teams, etc
Provides outreach to community and seniors through health fairs and visits to senior centers, etc
SUPERVISORY RESPONSIBILITIES
This job is a department manager supervisory position. Participates in Manager on Duty program; Supports on call daily; Develops, educates and enforces admissions policy and procedures which impact all staff.
QUALIFICATIONS
Requires experience in long term care. Good working knowledge of Medicare, Managed Care and other 3rd party payer clinical and financial guidelines. Working knowledge and experience with long term care admissions and discharge procedures. Must have a working knowledge of medical terminology. Must be able to deal respectfully with staff, residents, visitors and other health care providers. Must have working knowledge of computers and automated health care information systems.
