Assistant Client Care Coordinator

Position Summary
We are seeking a versatile and dedicated Assistant Client Care Coordinator to join our growing team. This hybrid role blends care coordination, scheduling management, and hands-on caregiving coverage for clients throughout the East Valley. As a new and expanding business, this part-time position is expected to quickly transition into a full-time role as we grow.

Assistant Client Care Coordinator Responsibilities
Scheduling Management
- Assist in creating and maintaining caregiver schedules in WellSky, ensuring proper coverage for all clients.
- Communicate promptly with caregivers and clients regarding scheduling updates or changes.
- Resolve scheduling conflicts and handle last-minute adjustments to maintain seamless care delivery.
- Monitor caregiver availability and proactively plan for future staffing needs.
- Ensure schedules align with client care plans and preferences.
Caregiver & Client Support
- Assist with client introductions and initial shifts to ensure smooth service starts.
- Communicate with clients and families to assess needs and adjust care as necessary.
- Support caregivers in navigating scheduling changes and maintaining work-life balance.
- Help track caregiver performance and provide feedback to leadership.
Hands-On Caregiving & New Case Support
- Provide direct caregiving services weekly, including personal care, ADL support, and companionship.
- Step in to cover shifts in cases of caregiver coverage needed or urgent client needs.
- Assist with onboarding new clients by attending initial shifts and ensuring a smooth transition.
Requirements
Experience: Minimum 2 years of caregiving experience (required). Previous scheduling, supervisory, and/or administrative experience highly preferred.
Caregiver Certifications: Caregiver Certification, CNA License, or Certificate of Completion from a Formal Training Program. Relevant experience in acute care settings is highly preferred.
Other Certifications: Current First Aid and CPR certifications required.
Background & Health Screening: Must pass a criminal background check, drug screening, DMV record check, and possess a negative TB test prior to employment. A fingerprint clearance card is also highly preferred.
References: Must provide at least two positive professional references from previous employers or supervisors that will be verified.
