Summary:
The Community Outreach Coordinator is the principle motivating force in efforts to facilitate the successful aging of clients through providing clinical, financial and social resources addressing their individual needs. This position will also act as the liaison between OMC, community groups, health related organizations, the general public and local businesses to educate and engage them in the Mission of the Ohio Masonic Communities. This individual will enhance existing relationships, develop new relationships and work closely as part of the OMC Resource Center team to develop mutually beneficial relationships to enhance the network of resources available to serve OMC Clients. As a Resource Center Coordinator, this position will provide compassionate care to those we serve.

Bachelor’s degree in Social Work, Gerontology or related field strongly preferred.

LSW, LISW/LISW-S or equivalent licensure preferred.

Experience designing and implementing programs preferred.

Demonstrable outstanding customer service experience required.

Experience in social services or related background required.

Experience working with older adults required.
*
Must have valid driver's license and be insurable.

Proficient in MS Office.

Ability to travel within assigned geographic territory.:

- Assist in navigating today’s complicated healthcare landscape, including providing answers to general Medicare/Medicaid, insurance, and other questions
- Help obtain products or services to enhance independence
- Coordinate with local programs that provide meals or transportation
- Connect individuals with the best accommodation, including a nearby campus of The Ohio Masonic Communities
- Connect individuals to home health, hospice or home-based medical services
- Provide connections and introductions to appropriate local agencies or caregiving resources
Summary:

This individual will enhance existing relationships, develop new relationships and work closely as part of the OMC Resource Center team to develop mutually beneficial relationships to enhance the network of resources available to serve OMC Clients. As a Resource Center Coordinator, this position will provide compassionate care to those we serve.
- Provide compassionate care coordination and case management services. Listen and evaluate each community member or client according to background, interests, abilities, and clinical, financial, and social needs, either telephonically or in-person and act as an advocate and a resource to clients’ and their families.
- Act as a partner in the aging process by answering all aging related questions: sharing information and connecting clients to products and services as needed.
- Bring forward OMC Emergency Financial Assistance requests as outlined by policy, as well as off-site assistance requests and follow up with recommendations on GLCF and Almoner’s Fund requests.
- Establish a schedule of home visits or calls with each client, per the client’s desired plan if applicable, or as necessary in all other situations.
- Track participation of clients and document activities and track referrals in RC database.
- Visit Masonic Lodges and other bodies in designated districts, doing presentations that promote awareness of the benefits OMC provides to the fraternal membership and the community at large.
- Responsible for professional networking, reaching out to targeted organizations and businesses (both existing and new) to develop mutually beneficial relationships, gain knowledge of available resources and assess community needs.
