Case Manager - FHPAP

Position SummaryThe Salvation Army Northern Division Anoka 360 Life Center is doing the most good by providing holistic, person-centered services for individuals and families in need. These services include food, clothing, utility, rent, and housing support, short- and long-term case management, spiritual support, and connections to community partners. The Case Manager for the Family Homeless Prevention and Assistance Program (FHPAP) guides participants in navigating housing and community resources through targeted case management services. The Case Manager empowers participants experiencing homelessness and housing instability to secure and maintain housing, increase their self-sufficiency, and connect to their communities. Additionally, the Case Manager provides direct assistance to participants and supports Coordinated Entry efforts within the county by completing participant assessments and engaging in community work.$23.00 / hour, 40 hours / weekThis is a time-limited position that will end on 9 / 30 / 2025.Essential Functions (approximate time spent) Provide participants with direct assistance. (45%)Provide participants with strength-based, person-centered, holistic case management, including completing Coordinated Entry assessments. (30%)Maintain up-to-date, accurate participant information in casefiles and through database entry. (10%)Engage in internal and external activities / meetings to promote and further The Salvation Army, FHPAP, and Coordinated Entry goals and objectives. (5%)Recruit qualifying participants for Pathway of Hope (POH) and promote social services programs with participants and throughout the community. (5%)Assist with seasonal and special events. (5%)Education and ExperienceBachelor's degree in relevant field preferred. Instead of a degree, a combination of education and experience may be considered. Must be licensed by the state board if the degree is in social work. Previous social services experience is required.Must pass background checks.Valid driver's license and meeting MVR requirements are preferred.CompetenciesFlexible interpersonal and communication skills.High level of honor in dealing with confidential information. Works with integrity, valuing honesty and diligence. Excellent organizational skills and attention to detail.Proficient with PC, Microsoft Office, databases, and standard office equipment.Working ConditionsMostly sedentary work, sitting for long periods and working on a computer. Professional environment; eligible for a remote-friendly work environment. Must be able to communicate clearly.Standard business hours with infrequent evenings and weekends.Occasionally lift 20 pounds.Supervisory ResponsibilitiesNone. Travel RequirementsMay occasionally drive agency vehicle locally to support the mission if MVR check cleared. May be required to use public transportation to support the mission if unable to drive on behalf of The Salvation Army.
