Educational Case Manager: Re-Entry Program
JOB SUMMARY
The SoulFisher Ministries is seeking an individual who performs highly complex case management work along with quality classroom instruction. This work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives, and procedures. Likewise, it involves developing, preparing, and instructing reentry classes, both during the pre-release process and post-release.
JOB RESPONSIBILITIES
- Research, develop and implement classes that support the overall growth and development of program participants.
- Facilitate and track all AGAPE reentry classes both pre-release and post-release.
- Admit new clients by reviewing records and applications; conducting orientations.
- Determine client’s requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations.
- Establish treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Monitor cases by verifying client’s attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements;
- Assist clients by obtaining additional resources; intervening in crises; and providing personal support.
- Maintain client records by reviewing case notes; logging events and progress.
- Communicate client progress to Reentry Program Coordinator and clinical team by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
- Prepare client discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements;
- Prepare reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics;
MINIMUM QUALIFICATIONS
- At least a Bachelor degree from an accredited college or university, preferably in Social Work, Psychology, Sociology, and/or Criminal Justice; At least two-years related work and/or practicum experience.
QUALIFICATIONS
- Excellent verbal and written communication skills and ability to network and connect with people at multiple levels
- Experience with project planning, budgeting and execution
- Proficiency with Microsoft Excel and other MS Office Suite products
OTHER REQUIREMENTS
- Able to lift up to 25 pounds
- Able to stand for long periods of time
- Able to sit for long periods of time
- Able to walk for extended periods of time and distance
Job Type: Full-time
Pay: $45,000.00 - $50,000.00 per year
