LPN - Home Health Professional

Job Summary:

We are seeking an experienced LPN to provide skilled nursing care to patients in their homes. This role requires strong assessment and communication skills, as well as the ability to work effectively with other healthcare professionals.

Your Key Responsibilities:

• Conduct thorough assessments of patients' physical, emotional, and social needs.
• Develop and implement individualized care plans in collaboration with RNs and other healthcare professionals.
• Administer medications, wound care, and other treatments as prescribed.
• Educate patients and families on healthcare needs and self-management strategies.
