Job Summary: The SSVF Case Manager is responsible for providing direct case management services to Veterans and their families experiencing homelessness or housing instability. This role involves conducting outreach, assessments, service coordination, and assisting Veterans in obtaining/maintaining housing and supportive services.
Essential Functions:
- Perform eligibility assessments to determine Veterans’ qualifications for SSVF assistance.
- Conduct community outreach to identify and support Veterans in need.
- Work collaboratively with government and community agencies to provide comprehensive services.
- Compile and maintain data for monthly reports and compliance requirements.
- Provide holistic case management, including intake, benefits assessment, goal setting, tenant education, advocacy, and referrals.
- Assist in maintaining case files with tenants' leases and related documentation.
- Conduct monthly housing inspections and collaborate with housing providers to secure stable housing.
- Ensure compliance with all applicable SSVF regulations before requesting assistance approvals.
- Connect Veterans and their families with community resources and conduct follow-ups to ensure the success of referrals.
- Assist eligible clients in completing SOAR applications.
- Attend community-based meetings when needed.
- Input data into the Homeless Management Information System (HMIS)
Qualifications:
- Associate’s or Bachelor’s degree in social work, human services, or a related field required.
- Proficiency in computers and case management systems.
- Strong interpersonal, communication, and organizational skills.
- Ability to work both independently and as part of a team.
- Knowledge of case management best practices, community resources, and housing regulations.
- Valid New York State driver’s license required.
Job Types: Full-time, Part-time
Pay: $31,000.00 - $35,000.00 per year
