Position Summary :

The Case Manager at TLC Care Center will play a vital role in coordinating personalized care and services for clients. This position involves assessing individual needs, developing tailored care plans, and ensuring seamless access to necessary resources and support. The ideal candidate will be empathetic, detail-oriented, and committed to advocating for clients’ well-being.

Key Responsibilities :

  • Client Assessment : Conduct thorough evaluations of clients' needs, goals, and resources to develop individualized care plans.
  • Care Coordination : Facilitate access to appropriate services and support, including medical care, housing, education, financial assistance, and mental health resources.
  • Client Advocacy : Serve as an advocate for clients, ensuring their needs are met and empowering them to achieve greater independence.
  • Progress Monitoring : Regularly review and adjust care plans to reflect changes in client circumstances, goals, or needs.
  • Documentation : Maintain accurate and up-to-date records, including case notes, assessments, and care plans, in compliance with organizational and regulatory standards.
  • Collaboration : Work closely with clients, families, service providers, and community organizations to ensure cohesive and comprehensive care.
  • Crisis Intervention : Provide support and intervention during crises, including identifying and mitigating risks to client safety or well-being.
  • Resource Development : Stay informed about available community resources, programs, and services to benefit clients effectively.
  • Training & Development : Participate in ongoing training, supervision, and professional development to enhance knowledge and skills.

Qualifications :

  • Bachelor’s degree in social work, psychology, nursing, or a related field (Master’s degree preferred).
  • At least 2 years of experience in case management, social services, or a related role.
  • Knowledge of healthcare systems, social service programs, and community resources.
  • Strong communication, organizational, and problem-solving skills.
  • Proficiency in case management software and Microsoft Office Suite.
  • Compassionate and empathetic demeanor with a commitment to client advocacy.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Certification in Case Management (e.g., CCM) is a plus.
  • Bilingual skills are an asset.
  • Working Conditions :

  • Office-based with some travel required to meet with clients or attend community meetings.
