About the Role
TPI Global Solutions is seeking a skilled Healthcare Case Manager to join our team. As a key member of our care services team, you will be responsible for assessing, planning, and coordinating the care of eligible members with complex healthcare needs.
Key Responsibilities:
- Conduct comprehensive assessments of members' needs and develop personalized care plans
- Collaborate with PCPs, occupational therapists, behavioral health professionals, and other stakeholders to ensure seamless care delivery
- Order and coordinate DME equipment as needed
- Educate and support members in managing their social determinants
- Provide travel and transportation services to facilitate face-to-face interactions
Requirements
- Registered Nurse licensure in DE with case management experience
- Proficiency in electronic documentation systems
- Strong communication and interpersonal skills
- Able to work independently and as part of a team
- Flexibility to adapt to changing priorities and deadlines
