The Family Nurse Practitioner (FNP) plays a critical role in providing comprehensive healthcare to patients across various age groups. Duties
- Prescribes medicines to patients for opiate addiction.
- Develop and implement individualized care plans based on patient needs and clinical guidelines.
- Monitors patients screens and lab results
- Manage acute care situations in outpatient.
- Provide health coaching to patients to promote lifestyle changes and improve health outcomes.
- Document patient encounters accurately in electronic health records, utilizing systems such as Aprima.
Skills
- Strong understanding of utilization management principles and standards of care.
- Excellent communication skills for effective patient education and collaboration with healthcare teams.
- Back round in addiction treatment
- Knowledge of health coaching strategies to support patient engagement in their own health management.
- Familiarity with electronic health record systems, particularly Aprima, is advantageous.
