The Director of Quality Improvement plays a vital role in driving initiatives to enhance care delivery, improve patient outcomes, and ensure clinical quality and safety standards are met. This leader oversees efforts to improve health outcomes for both patients seen at TrueCare and those assigned to a health plan but not yet engaged with our organization.
Key Responsibilities:
- Lead the Quality Improvement and Population Health Department, ensuring alignment with organizational goals and priorities.
- Develop and implement programs for preventive care and chronic disease management.
- Use risk stratification and patient segmentation to identify and prioritize interventions for high-risk populations.
- Drive efforts to enhance clinical processes, improve patient safety, and meet regulatory standards.
Requirements:
- Bachelor's degree in nursing from an accredited college plus a minimum of 5 years' clinical nursing experience.
- Previous experience leading quality improvement and/or change management processes and implementation cycles.
- Proven ability to lead multidisciplinary teams, including clinical, operational, and administrative staff.
