What are the responsibilities and job description for the Medical Case Manager position at Tulane University?
The Medical Case Manager for the Ryan White Tulane Total Health Clinic will be responsible for providing medical case management to eligible clients and will be responsible for the continuum of care to People Living with HIV/AIDS (PLWHA). The Medical Case Manager will support the adherence of clients to treatment and provide a safety net for the most vulnerable clients. In addition, the Medical Case Manager will work closely with our adolescent medicine linkage to care program to ensure all youth diagnosed with HIV are linked to HIV medical care. The Medical Case Manager is involved in community outreach, support for our community partner coalition, and youth empowerment activities.
Required Knowledge, Skills, and Abilities
- Excellent organizational skills and ability to handle multiple tasks independently
- Proficient computer skills; excellent working knowledge of Microsoft Office software applications (excel and word) Knowledge of electronic medical records systems
- Excellent verbal and written communication skills
- Ability to pro-actively work independently, ability to handle multiple projects simultaneously; ability to prioritize work, anticipate and meet established deadlines
- Working knowledge of available community resources and aptitude in locating new services when needed
- Ability to maintain confidentiality in all work performed
- Ability to maintain organization in all work performed
- Excellent interpersonal skills; ability to interact effectively and courteously with individuals of diverse cultural backgrounds
- Ability to work collaboratively with diverse populations

Required Education and/or Experience
- Master's Degree in Social Work, Health Science Education, Public Health or closely related social service field
- Two years of experience in healthcare
Preferred Qualifications
- Experience working with individuals living with HIV and AIDS, youth, adolescents, and adults
- Case Management skills to undertake patient education,access to community resources and crisis management as needed
- Experience working with community-based organizations,voluntary counseling and testing for HIV/ AIDS
- Strong interpersonal communication skills to continue to build strong relationships with local health department and other agency representatives
- Demonstrated ability to communicate in a culturally competent manner with ethically and sexually diverse individuals
