JOB SUMMARY
This position is responsible for providing quality services to high needindividuals living with HIV, ensuring that quality care outcomes are achieved, treatment adherence, documentation and performing related duties as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES

3. Monitor and follow-up on current immunological parameters and appointment adherence.
4. Conduct regular activities with clients that entail treatment adherence/disease progression, nutrition health, oral healthmonitoring, liver health, PREP access, health education and HIV disease transmission risk reduction.
5. Maintain a case load of a maximum of 60 clients.
6. Completes reports and documentation as required by VOASELA, Inc. and regulatory agencies/funding sources.
7. Prepares accurate and timely documentation for billing of services.
8. Referrals, as needed, to other providers within the Part A EMA service area for client access of services and tracking.
9. Keeps supervisory staff apprised of gaps in services and assists in efforts to provide needed services.
10. Participates in program related training, as required.
11. Performs related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
Must meet the service standards for Ryan White Part A-NOLA program by meeting the following criteria:
- Currently licensed social worker from an accredited SW program; OR
- Currently licensed nurse in Louisiana; OR
- bachelor’s degree (required) or master’s degree (preferred) in a human service-related field; AND 1 year of experience with HIV population.
Case management experience and working in a community setting is a plus. Must have excellent verbal and written communication skills.
Must have demonstrated willingness to be responsive to the needs of the target population and the ability to function as part of a team; can handle a flexible work environment; have strong decision making skills.
PHYSICAL REQUIREMENTS
The employee's work is performed at various locations where services are provided to consumers. Employee must be able to handle provide off-site case management to individuals in areas within the EMA, but outside of the city limits.
