This role involves regular travel between Jackson, Lenawee and Hillsdale County providing Home Based Care.
Overview:
The Certified Nurse Aide (CNA) performs patient care activities and other tasks to assist the primary care, therapy, and adult day health team members.
Thome PACE® serves Jackson, Lenawee, and Hillsdale counties providing participants with a wide range of medical and supportive services in our Day Center located in Jackson, Michigan or in the comfort of their own homes.

What you'll do:
- Assists in providing and/or maintaining optimal physical and emotional comforts to participants.
- Assists participants toward achieving maximum self-reliance and independence.
- Communicates with participants, families, staff, and the staff of other agencies.
- Participates in interdisciplinary team meetings.
- Provides basic ADL nursing care.
- Documents treatments/procedures, observations, nursing care, and participant activities.
- Reports changes in the participant’s condition to the pertinent interdisciplinary team member.
- Demonstrates active participation in QI processes.
- Participates in the process of teaching and learning with others.
What it Takes:
- Certified Nurse Aide (CNA) valid in the State of Michigan required.
- Possess current CPR certification or willing and able to obtain as offered by TP
- Must have reliable transportation-with valid driver’s license willingness to drive to Jackson, Lenawee and Hillsdale Counites.
- Minimum 1 year of experience working with elderly is required.
